Certified Medical Translation

The single most common medical-translation rejection at USCIS is a vaccination submission that the civil surgeon cannot use. Civil surgeons follow CDC technical instructions when reviewing foreign vaccination records, and those instructions are strict on what counts as documentation.

Acceptable vaccination documentation must come from a written record, either a personal vaccination card or a copy of a medical chart with entries made by a physician or other qualified medical staff, with month, day, and year for each dose. Self-reported vaccine doses without written documentation are not acceptable. The applicant carries the responsibility for providing reliable English translations of all foreign records to the civil surgeon. Our role on these submissions is the translation only, not the medical assessment. We render the foreign record completely and accurately, including the provider's name, every dose date, vaccine type and lot if shown, clinic stamp, and any handwritten margin notes. The civil surgeon then evaluates the translated record and transcribes the relevant entries onto Form I-693 itself.

Handwritten clinical notes, paper prescriptions, and pediatric vaccination booklets remain common worldwide, particularly from rural clinics, older hospital systems, and countries where electronic medical records were adopted late or unevenly. We translate these under the same certification standards as fully typed records.

The most frequent sources of handwritten medical records in our work include rural clinic notes from South Asia and sub-Saharan Africa, older European hospital records pre-EMR, paper prescriptions across Latin America, and pediatric vaccination cards used worldwide where the parent fills part of the booklet and the clinician fills the dose entries. Legibility is the practical concern. We recommend a high-resolution scan (300 DPI or higher) in even lighting, with the full document visible to its edges. If specific entries cannot be confidently read, they are marked as illegible in the translation with an explanatory note rather than guessed at. Guessing at clinical detail is the single fastest way to a translation rejection by a civil surgeon, insurer, or treating physician.

Age, water damage, sun exposure, and ordinary handling wear all degrade medical record legibility. We translate what is clearly visible and mark genuinely unreadable portions with explanatory notes. The translator never invents missing values or fills in clinical content that is not legible in the source.

Before ordering translation, check whether a fresh copy is available from the issuing facility. Hospitals, vaccination clinics, ministries of health, and EPI programmes can usually issue a duplicate of the original record, often faster and more reliably than attempting to translate from a damaged document. Replacement records are generally the cleaner path for I-693 vaccination evidence and US insurance reimbursement claims.

When translation from a damaged document is the only option, upload the clearest possible scan and include all visible portions: edges, stamps, signatures, and any apostille. If we cannot confidently produce a usable translation, we contact you before charging and discuss alternatives.

An apostille is an international authentication stamp issued by the competent authority in the country where a document originates. Under the Hague Apostille Convention, an apostille certifies the signature and seal on a public document for use in other member countries. For medical documents, an apostille typically authenticates a hospital seal, ministry-of-health stamp, or notarized clinical statement.

Whether an apostille is needed depends on the receiving party. Civil surgeons reviewing foreign vaccination records for Form I-693 generally do not require apostilles. US courts handling personal injury or probate matters that rely on foreign medical evidence may require apostilled records. If your medical document carries an apostille, the apostille itself is typically printed in the language of the issuing country plus French, per Convention requirements. The apostille text must be translated into English along with the medical record. We translate apostilles as part of standard medical orders at no additional charge.

Foreign medical records often list medications by local brand names that do not exist in the US market. A certified translation reproduces the brand name as it appears, paired with the international non-proprietary name (generic name) where the source clearly identifies it. We do not substitute the foreign brand with a US brand, because the receiving physician, insurer, or attorney needs to see what the source actually said.

Dosing units, lab reference ranges, and route abbreviations are also rendered as written in the source, not silently converted into US units. If a record uses milligrams per kilogram, the translation says milligrams per kilogram. If a lab reports values in SI units, the translation reproduces those values. The reviewing US physician, civil surgeon, or insurer is responsible for any unit conversion, not the translator.

Patient name spellings often differ across documents for the same individual. A name written in Arabic, Chinese, Cyrillic, or Devanagari script has no single correct English spelling. We render names exactly as they appear in the source document and do not change spellings to match other records held by the patient. If you have a preferred passport spelling, we can include a translator's note on the Certificate of Accuracy indicating the variations represent the same person.

One certified medical translation can typically be reused across multiple recipients. The same PDF given to a civil surgeon for I-693 review can be referenced again with USCIS in the I-485 file, with a US specialist before treatment, with a health insurer for reimbursement of foreign care, or with an attorney for personal injury or workers' compensation discovery. The Certificate of Accuracy is the element that makes the translation acceptable, not the specific destination it accompanies.

Some practical exceptions apply. Certain courts, insurers, and licensing boards prefer a translation dated within a recent window. If a specific recipient asks for a recently dated translation, a fresh copy can be issued at no additional cost for any order we originally completed. We also keep your file on hand for the standard retention period so a reissue is straightforward.

The most frequent causes of medical translation rejection, in approximate order of frequency: paraphrasing or summarizing clinical entries instead of rendering them word for word; missing dates, lot numbers, or doses on vaccination records; an incomplete, unsigned, or undated Certificate of Accuracy; substituting a US drug brand name for the foreign brand instead of preserving the source; converting doses or lab values into US units inside the translation; missing clinic stamps or physician signatures visible on the original; and patient name inconsistencies across the medical record, passport, and the I-485 file with no explanatory note.
