Certified Internship & Residency Certificate Translation

The first question every state medical board asks about postgraduate training is whether the program was accredited. The answer dictates the entire verification pathway, the documents needed, and whether the certificate alone is sufficient. The translation does not change the underlying classification, but accurate translation is critical so the board can read the source.

The Accreditation Council for Graduate Medical Education (ACGME) accredits all graduate medical training programs in the United States, including internships, residencies, and fellowships, for both MD and DO physicians since the 2020 single accreditation system. The Royal College of Physicians and Surgeons of Canada (RCPSC) and the College of Family Physicians of Canada (CFPC) accredit Canadian programs. Together these are the three accrediting bodies state medical boards specifically reference on Form PTA-PTB.

Foreign postgraduate training is not ACGME-accredited (with rare exceptions for ACGME-International programs). State boards that accept foreign training apply additional scrutiny: ECFMG primary-source verification through MyIntealth, state-specific time waivers, and direct verification with the issuing program or hospital. The translated certificate is the readable form of the source document, alongside which the verification layer operates.

State medical boards verify training duration against published minimums. California, for example, requires verification of receiving credit for 36 months of postgraduate training before granting a full physician's and surgeon's license. Most US state boards require 24 to 36 months for IMG applicants, with additional state-specific exceptions. A certificate that shows a vague academic year range without explicit calendar months can fall short on its face, even if the underlying training was sufficient.

In a translation we reproduce dates exactly as they appear on the source. We do not infer or normalize. Where the source uses academic-year shorthand (such as "2018-2019 academic year"), we preserve that phrasing literally and add no implicit calendar months. If a state board needs explicit start and end dates for verification, the program director typically issues a supporting letter with exact dates, which is also translated under the same standards.

Where multiple training periods stack (one year of internship, three years of residency, two years of fellowship), each certificate is translated separately and the bundle is reviewed together. Multi-certificate orders are still completed within the 24-hour window. Gaps in training history (research years, parental leave, military service) are flagged on the source as the issuing institution chose to phrase them, and we render that text faithfully.

Specialty names sit at the intersection of medical and academic translation. Most have clean US English equivalents that international medical curricula track closely. Cardiologie, Cardiologia, and Kardiologia all translate to Cardiology. Pediatria becomes Pediatrics, Medicina Interna becomes Internal Medicine, Anestesiologia becomes Anesthesiology. We use the standard US specialty nomenclature wherever a clean equivalent exists.

Some specialty names do not have a clean US equivalent, often because the source country's training framework groups subspecialties differently. Where that is the case, we preserve the source-language form and add an English translator's note clarifying scope. We never substitute a US specialty name that materially misrepresents the source training, because the receiving authority will check directly with the issuing institution and any discrepancy causes refusal.

Critically, the translation never asserts US board equivalence. Whether a foreign cardiology residency corresponds to ABIM Cardiology, whether a foreign general surgery residency satisfies ABS pathway, whether a foreign family medicine residency aligns with ABFM standards, these are determinations made by the relevant ABMS member board, not by translators. The translation faithfully represents what the source says. ABMS reviewers apply their own equivalence standards on top of the translated text.

Some state medical boards and credentialing offices request more than the completion certificate alone. Rotation logs, monthly logbooks, and program structure documents are often required to demonstrate that specific clinical exposures (for example, surgical case minimums, required medical specialties, or critical care months) were genuinely completed during the training period.

Rotation logs are typically formatted as multi-page tables listing month, rotation, hospital, and supervising attending. We render the table in English line-by-line, preserving every entry including supervising physician names and rotation hospital names. Where the log uses country-specific abbreviations (such as "C.G." for Cirugia General), we expand to the full English term and note the original abbreviation alongside.

Monthly logbooks (more granular than rotation summaries) capture daily or weekly clinical activity, often required for surgical residency applications and for hospital privileging at academic centers. These run to many pages and translation is priced at the same flat $22.99 per page rate. The bundle is delivered with a single Certificate of Accuracy covering all logbook pages, the certificate, and any supporting program director letter.

Apostille requirements differ across state medical boards and across destination types. Many US state boards explicitly require apostille on foreign-issued training certificates, treating them like other foreign public documents. The rule is set per-state, not federally, and applicants should check the state board addendum for their target jurisdiction.

An apostille is an international authentication stamp issued by the competent authority in the country where a document originates, certifying signatures and seals on a public document under the Hague Apostille Convention. For training certificate purposes, apostille is typically obtained from the foreign country's ministry of foreign affairs or designated apostille authority before the certificate is sent for translation.

If your training certificate carries an apostille, the apostille text itself must also be translated. Apostilles are typically printed in the language of the issuing country plus French, per Convention requirements. Training certificates are often issued years apart from the medical diploma and the medical license. Different documents may transliterate the same name differently, particularly for IMGs whose names are written in non-Latin scripts. Marriage, divorce, and court-ordered name changes between graduation and end of training add a further layer.

In a certified translation we render the name exactly as it appears on the source training certificate. We do not adjust to match the applicant's diploma, license, or current passport. If your internship certificate uses one transliteration and your residency certificate uses another, both spellings remain documented as-is in their respective translations. State boards, FCVS, and the GMC all expect to see the source documents at face value, which they then reconcile through their own name-on-credentials processes.

Where a formal name change has occurred, supporting documentation links the names. A translated marriage certificate, divorce decree, or court order accompanies the training certificate package. We can also include a translator's note in the Certificate of Accuracy where two transliterations refer to the same individual. Raising this at order stage produces the cleanest package for the state board or hospital review.

Training certificates are fixed historical records. A residency that ended in 2017 will not retroactively change in 2026, so a certified translation of the certificate does not become stale. The same PDF can be submitted to multiple US state boards, the Interstate Medical Licensure Compact, FCVS for portfolio inclusion, hospital credentialing offices, locum tenens agencies, and international regulators including GMC and MCC.

Each destination still independently verifies the underlying training. State boards verify ACGME/RCPSC/CFPC programs through Form PTA-PTB direct from the program director, and verify foreign training through ECFMG primary-source verification or direct hospital contact. The translation lets each authority read the document, but the verification layer is separate from the translation.

Multi-certificate bundles (internship, residency, fellowship) also reuse cleanly. Once translated, the bundle can be sent to FCVS for inclusion in the lifetime portfolio, then drawn from for state board licensure submissions across multiple states, locum tenens placements, and international regulator applications. Training certificate translation refusals at state medical boards, FCVS, and hospital credentialing offices follow a recurring set of patterns. Understanding these patterns helps applicants avoid the most common avoidable delays during what is already a long licensure timeline.

The most frequent causes, in order: a specialty name converted to a US ABMS specialty equivalent that materially differs from the source training; date formats normalized in a way that breaks regulator-side verification; missing apostille on the source where the receiving state board requires it; rotation logs or supporting program director letters left untranslated when the destination requested them; certificate omitted from the bundle entirely (for example, only the residency certificate submitted when an earlier internship certificate was also required); and translations prepared by the applicant or a co-resident in violation of independence rules.
